Coverage is the 51 filing jurisdictions — the 50 states plus the District of Columbia, which files and is regulated separately; the jurisdiction count reflects however many have posted filings so far. Jurisdiction figures are the median and range of carrier-level average rate changes as filed; they are not enrollment-weighted. Every headline range is the spread of carrier averages; the per-carrier “lowest/highest plan” columns are the only plan-level figures and run wider. Each filing is labeled Proposed (the carrier's request) or Final (the rate the regulator approved); where both come from the same source, the “Δ vs. request” column is the difference between them in percentage points, not a percent change. Where the approved figure is a state regulator’s own published average and the request is the carrier’s CMS filing, the two sit on different bases and no Δ is shown — the regulator’s posting, linked from the carrier name, states the change it made to the request it received. Dates beside a Final label are the regulator’s own approval date for that filing, where the source publishes one. While a plan year is part-approved, page-level medians and ranges blend approved and still-requested filings — use the Show filter to separate them.
